Alcohol Use Approval Form

If you plan to service alcoholic beverages (beer or wine only) at your event, you must complete this form AFTER receiving a reservation confirmation. The name of the OLCC server is required.

Submit the completed form at least 5 days prior to your event. You will be contacted only if questions arise.
